I understand I must arrive 10-15 minutes early for my appointment in order to get the full session time I have scheduled. If I arrive on time, or late, I understand the therapist can only give me whatever time remains of my appointment, and that I will pay for the full length of the session that I booked.
If I am not able to make a scheduled appointment. I agree to cancel or reschedule the appointment at least 24 hours in advance. I agree to pay $35 or 50% of the full session rate (whichever is greater) if I give less than 24 hours’ notice.
I agree to pay the full session rate if I give 2 hours’ notice or less, or if I miss an appointment without giving notice.
If within 24 hours of my season, I develop a contagious illness or have a sudden unplanned health or personal emergency rendering me unable to make my appointment. I will inform LBH Therapeutic Massage right away, and if you are unable to fill my vacancy, I will pay the cancellation fee, or session (if less than 2 hours notice), unless an exception is granted, only at the discretion of LBH Therapeutic Massage.
Pregnant Clients: If I go into labor within 24 hours of an appointment I agree to notify LBH Therapeutic Massage, and you will waive the cancellation fee. If I go into labor and do not notify LBH Therapeutic Maassage, and I miss my appointment, I agree to pay the full session rate.
I understand that I am still responsible for my appointment until I hear back from a staff member confirming they received my email or phone call requesting cancellation/rescheduling.
I understand that massage therapy is provided for stress reduction, relaxation, relief from muscular tension, and improvement of circulation and energy flow.
If I experience pain or discomfort during the session, I will immediately inform my therapist so that pressure/strokes can be adjusted to my level of comfort. I will not hold my therapist responsible for any pain or discomfort I experience during or after the session.
I understand that my therapist wants my HONEST feedback- positive or negative- and doesn’t take offense to it.
I understand that the services offered today are not a substitute for medical care. I understand that my therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness.
I affirm that I have notified my therapist of all known medical conditions and injuries.
I agree to inform the therapist of any changes in my health and medical condition. I understand that there shall be no liability on the therapist’s part should I forget to do so.
I understand that massage is entirely therapeutic and non-sexual in nature.
If any behavior is considered inappropriate–including but not limited to off-color comments, sexual innuendo and/or rude requests–the session will be terminated immediately, and payment in full will be expected.
I understand that, because massage therapy work involves maintained touch and close physical proximity over an extended period of time, there may be an elevated risk of disease transmission, including COVID-19.
Locations outside a 25 mile radius of Columbia, MD will have a separate fee of $2.00 a mile outside of the 25 mile radius.
When convenient free parking is not readily available, the client will cover parking expenses
*Prepayment of all services is required to hold the appointment.
